Online toolkits which can support community workers, partners, families and carers to provide better assistance to people living with dementia and other chronic conditions.
These online toolkits have been developed by Dementia Australia Vic to support community workers, partners, families and carers to provide better assistance to people living with dementia and other chronic conditions.
The series includes:
- Dementia and Diabetes
- HIV And HIV-Associated Neurocognitive Disorders (HAND)
- Dementia And Stroke Or Coronary Heart Disease
The resources contain information on the causes, symptoms and strategies for managing the conditions, along with a range of help sheets, case studies and educational videos. There are also links to community services and programs for more information and support.
